Dear Neighbor:
We are asking people with chronic diseases, including heart diseases, stroke, cancer, respiratory diseases, diabetes, and kidney diseases, family/non-professional caregivers, providers/professionals, and community stakeholders that have an interest in supporting research efforts combating the COVID-19 pandemic to participate in a survey-based research project.
Our survey seeks to understand individual and community opinions and ideas about adherence to COVID-19 mitigation measures, testing, and vaccine uptake in an effort to map out the best processes to help our communities. There are no correct or incorrect answers on this survey. We ask that each participant answer the questions to the best of his/her ability and understanding. All information acquired will be helpful. This survey will inform current and future research efforts.
